Community Outreach Consultancy Ltd provides a dedicated Case Management Service supporting adults and children through rehabilitation to reach their full potential. We look after spinal injuries, brain injuries, cerebral palsy and many other complex conditions and disabilities, often requiring 24/7 support at home. We work with all professionals to support and secure care to meet the client’s individual needs. Our professional Case Managers have a wealth of experience and knowledge in complex care in the home, including spinal injuries, brain injuries and ventilated clients. Many are qualified in nursing (adults and children), Physiotherapists or Occupational Therapy.

TRAINING ACADEMY - The key to our success is the in depth training and supervision we provide for the client and their care team. Care staff are specifically trained on the individual clients care regime. This includes clinical procedures, professional boundaries, mandatory training, including manual handling, Common Induction Standard, record keeping and personal care tasks. We manage regular Staff supervision, staff appraisals, team meetings and monitoring which prevents any issues escalating and maintains good communication for the client and the care team


Community Outreach Consultancy Ltd. ensures that the client has a care team to suit their individual care requirements. This includes a range of specialist home-based nursing, personal and practical care for children and adults with complex and life-limiting conditions. The service provided enables parents, families and carers to have some respite from their day to day caring responsibilities. Community Outreach Consultancy Care Managers are qualified nurses (RN’s, RSCN’s) with experience and knowledge in complex care in the home, including spinal injuries, brain injuries and ventilated clients. Our success has been providing high quality bespoke care regimes tailored specifically to the clients’ clinical and personal requirements. We offer the client a ‘person centred’ managed service to enable flexibility, choice and individually tailored care programmes to suit their changing lifestyle.

Community Outreach Consultancy Ltd are registered with the Care Quality Commission as nursing and domiciliary care providers. This enables clients to feel confident and reassured that they will have the best skilled care team support to meet their individual requirements. Community Outreach Consultancy is not an agency. We are able to provide sustainability and continuity of care by incorporating a high level of supervision, training and monitoring of all care staff.

THE CARE TEAMS - All staff are recruited to deliver specialised care and will possess a range of care skills, qualifications, experience, knowledge and competencies to deliver high quality care for our clients. Team Leaders are qualified children’s/adult nurses (as appropriate) with leadership skills and experience. All Disclosure Barring Service (DBS) checks are carried out prior to engagement.

Health Carers/Carers/Support Workers are QCF/NVQ level 3 or above and have experience working with children and adults with complex conditions.

The care teams recruited are trained to meet the clinical and personal care needs for each client. Once the care team members are recruited we continue to support with management, supervision, appraisals and team meetings. On-going training is provided on a one to one basis or as a group.


We provide Liability reports written by Registered Nurses with relevant experience and knowledge. We identify if best practice in nursing and personal care has been carried out in accordance with the professional codes and legislation.

ACCREDITATION - Community Outreach Consultancy Ltd is registered by the Care Quality Commission. Memberships include Nursing Midwifery Council (NMC) Royal College of Nursing, Case Manager Society U.K. (CMSUK) and British Association of Brain Injury Case Managers (BABICM), National Skills Academy.
